Blood Pressure Self-Monitoring Program

The program is designed to support participants in realizing these benefits:
• Reduction in blood pressure
• Better blood pressure management
• Increased awareness of triggers that elevate blood pressure
• Enhanced knowledge to develop healthier eating habits

Through this four-month program and with the support from our Healthy Living Staff, participants will aim to:
• Measure and record their blood pressure at home at least two times per month, gaining increased awareness of triggers that elevate blood pressure
• Attend two personalized consultations per month with a Healthy Heart Ambassador at the Y
• Attend monthly Nutrition Education Seminars

Benefits
• Joining the Blood Pressure Self-Monitoring program is an investment in health and a commitment to reducing risk for heart attack or stroke.
• Participants develop the habit of self-monitoring to lower or better manage blood pressure.
• Investment of personal health to reduce risk of heart disease.
• The fee for the program ensures the program is sustainable and can benefit those who have high blood pressure.

To participate in the program, participants must be:
• At least 18 years old
• Diagnosed with high blood pressure
• Have not experienced a recent cardiac event
• Do not have atrial fibrillation or other arrhythmias
• Are not at risk for lymphedema

Participants pay a one-time fee that covers the cost of the four month program.

Participants need to complete two forms before we can schedule a check-in appointment.

Participants will need a blood pressure monitor for tracking. We do offer a Blood Pressure Monitoring Table upstairs on our Wellness Floor. 

Non Member price: $50 (all 4 months are included)
YMCA Member price: $20 (for all 4 months)

Monthly Nutrition Seminar 

  • Only need to attend one seminar a month of the four months:
    • Wednesday at 5:30pm in the Community Room: 09/25/2024, 10/23/2024, 11/20/2024, & 12/18/2024 – 45min
    • Thursday at 1:00pm in the Community Room: 09/26/2024, 10/24/2024, 11/21/2024, & 12/19/2024 – 45min

Our monthly nutrition seminars highlight how food is related to blood pressure management and will provide the knowledge and skills to make heart-healthy choices. Topics covered include reducing sodium intake and heart-healthy eating.

Each 45-minute seminar will offer an opportunity for participants to take his/her/their own blood pressure with coaching on proper technique from a Healthy Heart Ambassador.

Members can attend these seminars for FREE and do not have to register for the 4 month program; although, we strongly recommend educating and empowering individuals about their heart health. Nonmember cost is $5.
